We currently have contracts with the insurance plans shown below. However please be aware that not every doctor is credentialed on every plan: We intend to get most of our doctors on most of the same plans, but there may always be variances. Aetna PPO/HMO

• Aetna PPO: All NWDC physicians accept new Aetna PPO patients, without restriction.

• Aetna HMO: Effective February 1, 2008, all primary care doctors are on Aetna HMO, but are not accepting new Aetna HMO patients. Our rheumatologists, Drs. Carmen Perez-Masuelli and Leslie Wilson, ARE continuing to accept new Aetna HMO patients in referral from other doctors.

Aetna TRS

Blue Cross/Blue Shield - PPO, PAR plan & BlueChoice Solutions

CCN

Cigna - Northwest Diagnostic Clinic is in-network for Cigna patients effective May 1, 2008.

First Health

Humana - We have signed a contract with Humana and it is now in effect. Thanks to those of you who indicated to your employer that you wanted NWDC in your network.

Medicare/RR Medicare

• Contact our scheduling desk to learn which NWDC primary care physicians are still accepting new Medicare patients.

• Almost all NWDC physicians are accepting new SelectCare TexanPlus members under the Medicare Advantage program. For more information about this option, please click here.

PacifiCare - We do not currently have a contract with PacifiCare. However, because of the merger of PacifiCare and UnitedHealthcare, we may be accepting certain plans labeled as PacifiCare, including PacifiCare Signature Elite PPO.

Unicare PPO/HMO

UnitedHealthcare PPO/HMO - We accept patients covered by all UnitedHealthcare products.
